Step 1: Set up Spruce Health Messaging App -  (2 minutes)

Please use the link https://spruce.care/mindbalancementalhealthcare612 to set up your Spruce Health Account. This is a secure messaging app for all communication. Please use this for all communication going forward.

Step 2: Client Registration Form (2 minutes)

You will be sent the client registration form once you set up your Spruce Health App account. Please complete the Registration form and send it back to me using the Spruce Health App.

Step 3: Client portal Set Up (20 minutes)

You will receive an email invitation to set up your client portal. This is where you complete all paperwork, including the intake paperwork, upload payment information, and schedule all appointments.

Once you finish the intake paperwork, please schedule your intake appointment.

No Surprising Act/Good Faith Estimates - out of network/self(private) pay

Good Faith Estimates (GFE) are part of federal regulations that went into effect January 1, 2022 as part of the No Surprises Act aimed at protecting consumers from surprise medical bills for services/items provided by out-of-network providers/facilities by ensuring they are made aware of the estimated costs associated with seeing an out-of-network provider. A notification of the GFE along with GFEs will be to provided to any person who is self-pay for the following reasons: uninsured/out-of-network/chooses not to use insurance benefits.  This will also be discussed with you at your initial consultation appointment, and before any services are scheduled.
